Monday – Friday, 8.30am – 5pm.  At evenings or weekends please contact the Crisis Team directly on 01202 652000.

How can I refer?

Referrals can be made via letter, telephone, electronically or fax, directly to the CMHT by the GP or any Health or Social Care Professional, including:

Members of the Primary Care Team

Local Authority Staff

Staff in other Specialist Mental Health Services

Primary Care Mental Health Teams (inc IAPT)

Staff from General (Acute) Hospitals

Staff in Tier 3 Addictions Services

What functions/treatment and therapies are offered?

There are 8 Adult locality teams, with CMHT Bases located in a variety of geographical areas, each one looking after different communities.  

The aims of each Community Mental Health team are to promote Wellbeing, better Quality of Life and Recovery through:-

Providing high quality, evidence based treatment and care for Service Users

  • Providing support that enables each Service User to achieve their individual goals and aspirations
  • Informing Service Users and carers about options for treatment and hence enable treatments to be a partnership guided by Service User choice
  • Providing information and support for Service Users and carers
  • Helping service users in them accessing a range of mainstream services, e.g.

o Housing

o Employment or meaningful occupation

o Stable relationships

o Primary Health Care etc.

  • Promoting holistic care taking account of physical as well as mental good health
  • Ensuring that Service Users and carers are treated with respect and dignity
  • Ensuring that care is offered in accessible locations and in a timely manner

Working in an environment that safeguards vulnerable people

Our friendly, professional staff are dedicated to supporting people at home and preventing unnecessary admissions into hospital and so work closely with General Practitioners, Social Services Day Centres, Care Homes and our Day Hospital services.

Which types of illness/medical condition does the service treat?

The service provided by the CMHTs will focus on Service Users with a significant mental disorder for Service Users aged 18 and over and include individuals with;

  • Schizophrenia and psychotic disorders and delusional disorders
  • Affective disorders such as mania, manic depression and moderate / severe depression
  • Eating disorders
  • Organic Mental Disorder
  • Significant mental health problems following childbirth
  • Significant neurotic, stress related and somatoform disorder, such as phobias, anxiety disorder, post-traumatic stress disorder and obsessive-compulsive disorder which have not responded to previous appropriate treatment
  • Personality disorder co-existing with the above diagnoses and/or personality disorder that causes significant distress or risk to the Service User or others and where there may be a benefit from specialist expertise
Who will I see?

To provide the most effective care, our teams are a mix of different Health and Social Care professionals.

As a patient, how do I access services?

If you would like to see a member of your local community team and have not used this service before, please ask a local health (such as your GP) or social care professional to refer you.

If you are an existing service user, please contact your Community Mental Health Team, Monday – Friday, 8.30am – 5pm.  At evenings or weekends please contact the Crisis Team directly on 01202 652000.

How can I get involved / what support groups are available?
