The Memory Assessment Service provides assessment for people experiencing memory problems, ensuring an early diagnosis of dementia is made if this is the problem and treatment provided where appropriate to help slow down memory loss.

The service is open Monday – Friday, 9.00am – 5.00pm.

The service provides assessments to Dorset residents who are concerned about their memory, where appropriate make a diagnosis of dementia and offer anti dementia drug treatment in line with NICE Guidelines. Patients are initially seen in a Memory Clinic by a Community Mental Health Nurse for assessment before seeing a Doctor when required. The service will stay in touch with those people diagnosed with dementia who are receiving anti -dementia medication , signpost them on to other local support services and monitor any treatment plan initiated. Memory clinics take place at a range of community settings including GP surgeries.

Memory Assessment Nurse. If required, you may also see a doctor (Psychiatrist).
Individuals who wish to meet with the Memory Assessment Service can simply visit their GP, who will take a blood test and ensure relevant information is provided to the Memory Assessment Team. All service users will then have a first appointment with a Memory Assessment Nurse within four weeks of their GP making the referral.
