Support General Practitioners with management of patients with long term conditions who are at risk of frequent hospital admission.

What are the service opening times?

From 1st April 2012 : 08.00am - 20.00pm.  Please contyact your Local Team for further i nformation.

How can I refer?

Services accessible via GP referral or direct to Community Nursing Team.

What functions/treatment and therapies are offered?

The general function is the overseeing of the long term condition and management of this. 

Which types of illness/medical condition does the service treat?

Any long term condition.

Who will I see?

You will be seen by a Community Matron who is a registered Nurse with additional training in Clinical Examination Skills, Case Management and Prescribing. 

As a patient, how do I access services?

You can access this service via your GP or Community Nursing Team.

How can I get involved / what support groups are available?

Your local GP Practice/Surgery will advertise any support groups.
