End of life care includes palliative care. This is for patients with an incurable illness. Palliative care seeks to make the patient as comfortable as possible by relieving pain and other distressing symptoms while providing psychological, social and spiritual support for the individual, their family and/or carers.
The Generalist Palliative Care Service is a team of professionals providing expert end of life palliative care to patients in their own home and supports their families and carers. It is available to patients over the age of 18 with a life limiting disease (cancer or non-cancer) who are registered with a Bournemouth or Poole GP. The service provides end of life care and complements other palliative care and the long term conditions service.
Patients may receive palliative care early in the course of their illness together with other therapies, such as chemotherapy or radiation therapy.
The Specialist Palliative Care Service is based at Forest Holme in Poole. It consists of a team of professionals who are highly skilled in palliative care services. They provide support for people living with an advanced, life threathening, complex illness and for their carers. The service works with GPs, District Nurses, Consultants and other hospital and community staff to make sure patients receive the best possible care.
Referrals will be made via a hospital, GP or other health professional.
