To facilitate patients return to independence wherever possible, and to help the patient and their family adjust to the life changes associated with stroke.

What functions/treatment and therapies are offered?

The service provides stroke specific support, education and advice to patients who have suffered a stroke, their families and carers.  Wherever possible initial contact is made with patients and their families during the patients stay in hospital.  Arrangements are then made to visit at home after discharge to assess any further health and social needs.
