What functions/treatment and therapies are offered?

There are 8 Older People's locality teams, with CMHT Bases located in a variety of geographical areas, each one looking after different communities.  They provide a wealth of valuable services such as assessments, treatments, home visits, group and individual therapies.

Our friendly, professional staff are dedicated to supporting people at home and preventing unnecessary admissions into hospital and so work closely with General Practitioners, Social Services Day Centres, Care Homes and our Day Hospital services.

Which types of illness/medical condition does the service treat?

The Community Mental Health Team (CMHT) is comprised of trained staff who are experienced in working with people who have mental health problems such as Depression, Anxiety Disorders, Schizophrenia and Dementia. Teams see people in a variety of settings, most often in their own home, Outpatients or at the CMHT base.

Who will I see?

To provide the most effective care, our teams are a mix of different Health and Social Care professionals including Psychiatrists, Nurses, Occupational Therapists, Social Worker, Support Time Recovery Workers and administrative staff

As a patient, how do I access services?

If you would like to see a member of your local community team and have not used this service before, please ask a local health (such as your GP) or social care professional to refer you.

If you are an existing service user, please contact your Community Mental Health Team, Monday – Friday, 9.00am – 5pm.  At evenings or weekends please contact the Crisis Team directly on 01202 652000.
