Community Mental Health Teams (Older Peoples - North & West Dorset)

What does it do?

The teams provide a comprehensive communmity mental health service for people over the age of 65 with mental health needs and for younger people with organic mental health needs (eg dementia).

What are the service opening times?

9.00am to 5.00pm, Monday to Friday.

Open referrals can be made via letter, telephone, electronically or fax, directly to the CMHT-OP.

What functions/treatment and therapies are offered?

There are 6 community teams, with CMHT-OP bases located in a variety of geographical areas, each one looking after different communities. They provide :

  • A comprehensive assessment including diagnosis.
  • Memory assessment and diagnosis.
  • Assessment for Alzheimer's medication and prescribing where appropriate.
  • Advice on meeting mental health needs.
  • Assessment of risk.
  • Formulation and implementation of integrated care plans.
  • Review of care plans.
  • Liaison, support and advice to other services on the management of mental health needs.
  • Access to psychological therapies where required.
Which types of illness/medical condition does the service treat?

Functional or organic mental health needs.

A member of the multi disciplinary team, who may be :

  • Psychiatrist.
  • Nurse.
  • Occupational Therapist.
  • Psychologist.
  • Support Worker.

Self referral, or via your GP or another Health or Social Care Professional.

Early Memory Loss Groups.

Carers Support Groups.

Carers Education Groups.

Memory Cafes.

Anxiety Management Groups.

Access to various activity groups.

Access to day care, including day hospital services.
