COVID-19 service update: normal service
Manage your health condition from the comfort of your own home
Telehealth allows us to monitor some patients remotely, in their own home, reducing the need for visits from healthcare professionals or trips to hospital.
We can install technology in people’s homes which will allow them to take daily readings of their condition, and feed this information back to our staff at the touch of a button. If the readings show any potential issues, we will contact the patient to discuss further, liaising with their GP, specialist nurse or community matron as appropriate.
Telehealth is currently available to patients in Dorset with Chronic Obstructive Pulmonary Disease (COPD) and Chronic Heart Failure (CHF), though we hope to offer the service to people living with other long-term conditions in the near future.
The system is easy to use and allows people to take a range of different health readings, including:
- temperature
- weight
- blood pressure
- oxygen saturation
- pulse.
Telehealth can be provided for up to twelve months, until the patient is confident in self-managing their condition. If you would like to use this service, please speak to your GP or other healthcare professional – they can put you in touch with us by using a simple referral form.
When we have your referral form, a member of our Telehealth Hub team will contact you to discuss your condition and suitability for the service. Telehealth equipment will be installed by our contractor, Baywater, within five5 working days of a referral being accepted.
