We provide nursing care and treatment for people with complex, long-term conditions in their own homes.
Our community nursing teams support house-bound residents with a wide range of mental and physical health needs, from wound management and diabetes to kidney failure and heart or lung conditions.
They work closely with GPs, hospital staff, consultant specialists and social care workers to identify a person’s individual care needs and co-ordinate the different services they receive.
Our community matrons and district nurses also provide information and advice to help patients (and carers) to manage their health confidently and independently at home. This helps to avoid complications associated with immobility, disability or existing illness, and to minimise hospital admissions as much as possible.
