If you have received a letter from us asking you to provide details of your child’s development history, please click on the relevant link below to access this. Once completed, this information will be securely submitted to our administration team and added to your child’s health record. This is the important first step in the assessment of your child and the Speech and Language Therapist will review this information prior to your appointment.
If you have not received a letter requesting this information, please refrain from doing so. Any information submitted which has not been requested will be deleted from the system.
