Terms and conditions of a wheelchair loan

All equipment issued remains the property of the Dorset Wheelchair Service and is loaned to you under the following conditions.

Do

Keep your wheelchair clean and in good working order.

Keep your wheelchair protected from damage, loss or theft and notify us immediately of any such occurrences.

Follow the manufacturer’s recommended usage for your wheelchair, especially the safety aspects, as stated in the user handbook.

Make your wheelchair available for repair or inspection, if requested by the service.

Take part in a review or monitoring process if contacted by the service.

Tie down your wheelchair if used in transport (if crashed tested), in accordance with the wheelchair manufacturer’s recommendation found in the user handbook.

Accept that your wheelchair may be withdrawn for any of the following reasons:

  • safety (yours, the public or the equipment)
  • you no longer meet the criteria for provision of a wheelchair
  • failure to comply with these terms and conditions.

Do not

  • let anyone else use your wheelchair
  • use your wheelchair for any other purpose other than that for which it was provided
  • alter your wheelchair or fit any attachment to it, without our agreement
  • sell it or dispose of your wheelchair for any reason.

Please inform the service on 01202 892874 if:

  • any of your personal details change such as address, GP, weight, medical condition.
  • you no longer require your wheelchair.
  • your wheelchair is lost or stolen, involved in an accident or mishap.
  • you become a permanent resident in a care home.

 

Repairs

Opening hours

Monday-Friday: 8am to 5pm.

Saturday: 8am to 12pm (emergency only).

An answer phone is available for messages out of hours and we will endeavour to return any calls within one working day.

When contacting the service about a repair please provide your name, address and details of exactly what the problem is with the wheelchair. If more than one wheelchair has been provided by the service, please state which wheelchair requires the repair.

This service is free of charge for reasonable wear and tear on wheelchairs provided by the NHS, except in cases of repeated misuse, neglect or wilful damage.

The service does not provide roadside repairs or recovery home.

It is not always possible to repair a wheelchair immediately and a loan wheelchair may be issued but will not necessarily be the same specification.
