COVID-19 service update: phone support available, plus limited outreach work.

We provide intensive, community-based support for people who are homeless and struggling with a physical illness or have a mental health condition.

Our Nurse Practitioners offer assessment, care, treatment and referral for people who are homeless with a physical health need. They are able to offer general health assessments, wound care and dressings, GP registration, vaccinations, a range of health testing & health promotion.

Our Mental Health Practitioners offer assessment, care and treatment for people with significant complex mental health needs who require regular and intensive support and are not able to currently access mainstream mental health services.

The team all work in an outreach approach and will visit you where you’re most comfortable to make a full and comprehensive assessment of your needs and risk, and prepare a care plan tailored to you. We will provide support, for as long as you need it, until your current difficulties are resolved – or from the point where you’re able to access mainstream health services provided by your local CMHT, GP or any other mainstream health service. If we cannot resolve your problems in the community, care may need to continue on one of our hospital wards or we may keep working with you for a short period more but this will always be discussed with you first.

Our specialist Homeless Health Team includes mental health nurses and registered general nurses. We will see you where you are currently sleeping, in a public place, day centres or at a health building. Our aim is to deliver health care to service users during a time when access to mainstream services is challenging.

The team works closely with mainstream primary and secondary physical; and mental health services, the rough sleepers teams in Dorset, local authority, police and a range of third sector and voluntary organisations across Bournemouth, Poole and West Dorset.

Support is available five-days a week. If you require support from our service we accept self-referrals as well as referrals from any other agencies who maybe working and supporting you.
