Community Matrons work with patients with long term health conditions and complex needs.

What are the service opening times?

Currently 8am - 9pm; extending to 7am to 10pm from 1st April 2012

What functions/treatment and therapies are offered?

Community Matrons are able to:

  • Ensure the best care for patients with chronic and complex health conditions.
  • Enable patients to take greater control of their condition and lifestyle.
  • Provide patient-centred treatment and support for patients and their carers.
  • Reduce unnecessary hospital admissions and GP visits.
  • Co-ordinate and plan delivery of Health and Social Care services.
  • Work with Palliative Care teams and District Nurses to manage End of Life care.
  • Help improve the quality of life of patients and carers.
  • Liaise with and work with Intermediate Care Services and General Practices.
Which types of illness/medical condition does the service treat?

This service can be accessed by patients over 18 years of age who have a confirmed diagnosis of a long term health condition.  

Who will I see?

A Community matron or a member of the intermediate care team making an assessment on their behalf

The service can be accessed by referral from your GP.  You can also refer yourself by calling 01202 711535.
