School age immunisations registration

Dorset Healthcare has partnered with Inhealthcare to provide a fully digital immunisation service for school age children in Dorset. This means you can provide consent for your child to receive their immunisations online at your convenience on any device, anywhere and at any time.

Following feedback from parents and schools we have simplified the process for providing consent for the 2020/21 academic year. This is now a simplified one-step process.

How to complete the online consent form

You will receive an offer letter from SAIS offering your child their vaccination. This will include a link to the online consent form and a code that is unique for your child’s school (or their LEA, if home educated). If you have children at different schools you will receive separate offers with different codes. This code is used to allocate your child to the correct session.

When completing the online consent form you will be asked to provide an e-mail address which we will use to communicate with you about your child’s immunisation. To ensure that we use the correct e-mail address, we will send you a verification code which you will need to enter to complete the online consent process. This e-mail will come from our partner organisation, Inheathcare.

If you are unsure about the answer to any of the health questions e.g. about previous vaccinations please check with your GP.

If we have any questions regarding the information you have provided one of our nurses will contact you, please therefore provide a daytime telephone number if possible.

If your child is school educated, the date by which the online consent form must be completed is shown on the offer letter.  Please ensure that you complete the online consent form in good time to ensure your child can be vaccinated at school. You can still complete consent online after this date but will be offered a clinic appointment at a catch-up community venue instead.

Please note: You must have parental responsibility for the child to provide consent for them to be immunised.

What if my child has additional needs?

The online consent form includes a question regarding additional needs. If your child has additional needs please tell us and one of our trained nurses will contact you to discuss whether any special arrangements need to be made for your child to have their immunisation.

What if I do not want my child vaccinated?

We will only immunise a child if we have received consent from somebody with parental responsibility.

However if you decide you do not want your child to be immunised, we recommend that you still complete the online consent form so that we are aware of this when we visit school. Providing a reason will also help us plan and improve the service.

What if my child becomes ill after I complete the form?

If your child has a serious illness after you complete the online consent form please let us know by e-mailing dhc.immsenquiries@nhs.net or phoning 01425 891162.

Our operating hours are 9am - 5pm, Monday to Friday. You can leave an answerphone message outside of these hours

What if I am unable to use the online service?

If you are unable to complete the process online because you do not have an e-mail address or you do not have internet access, we can register your child and complete the form with you over the phone. Please ring us on 01425 891162 between 9am - 5pm Monday to Friday and one of our team will be happy to assist.
