Who can I nominate?

We want everyone to take the chance to say thank you.

Anyone, including patients, carers and volunteers, can submit a nomination and you can nominate as many people as you want.

You can nominate your nearest and dearest team mates or someone you have received great care from, who are doing brilliant things for patients or their colleagues.

An individual or a whole team can be nominated.

Tip: If you are nominating a team, have all the names of the team members to hand when completing the nomination form.  We wouldn't want anyone to miss out.

Duplicate nominations will not be accepted or processed ie: same person/team for the same award.

Nominations for yourself or your own team will not be accepted or processed.
